Can someone die while they're on a ventilator at the hospital?

it can happen, and it's a scary thought. while i was in the icu i managed to pull my breathing tube out in my morphine stupor and i laid there suffocating for 2 or 3 minutes before a doc got me re-intubated and sedated. if no one heard my alarms going off i would have completely suffocated.
